N2  - The treatment of Type 2 Diabetes in the health care system can undergo a transition among health care facilities which may lead to drug-related problems. Pharmacists have clinical knowledge and skills to conduct pharmacotherapy workup and enable patients to achieve therapeutic goals. This study aims to prove a comprehensive medication management model to be implemented in primary care and to evaluate the effect on patient empowerment and HbA1c.
