Pharmacists have clinical knowledge and skills to conduct pharmacotherapy workup and enable patients to achieve therapeutic goals. This study aims to prove a comprehensive medication management model to be implemented in primary care and to evaluate the effect on patient empowerment and HbA1c. This study used the experimental design on the implementation of a comprehensive medication management model in diabetic patients who were divided into the intervention and control groups with a total of 42 patients at the Surabaya Primary Health Care using the form of patient empowerment assessment.
